About Blepharoplasty
We often describe the eyes as the windows to the soul, but over time, they can become the first place where signs of aging appear. Upper Blepharoplasty
Upper blepharoplasty specifically addresses the sagging skin that can create a heavy or hooded look on the upper eyelid. As the skin loses its elasticity, it can descend, drooping below your lashline and occasionally obstructing your peripheral vision. During an upper eyelid lift, Dr. Bradley makes a precise incision hidden within the natural folds of the eyelid. Through this access point, he removes excess tissue and tightens the skin, restoring the eyelid’s clean, youthful crease. In addition, Dr. Bradley will assess whether you have drooping of your upper eyelid, known as eyelid ptosis. In this condition, the upper eyelid is lower than it should be, like a window shade hanging too low. This condition may happen with aging. Dr. Bradley will evaluate you for this condition and, if present, will correct your eyelid ptosis during your upper blepharoplasty procedure.
Lower Blepharoplasty
Lower blepharoplasty is designed to treat the “bags” or puffiness beneath the eye. These often occur because the fat that cushions the eye starts to bulge outward as the surrounding tissue weakens. This manifests as a perpetually exhausted look, even after a full night’s sleep or a restful holiday. During this procedure, Dr. Bradley addresses these fat pads by either removing a small amount or repositioning them to smooth out the transition between the lower eyelid and the cheek. (3) This creates a seamless, youthful contour that eliminates the shadow-casting hollowness beneath the eye.
With aging, and in some younger patients who show this family feature, a tear trough may develop that separates the lower eyelid from the upper cheek. This tear trough prevents a smooth, youthful transition from the lower eyelid to the cheek. If present, Dr. Bradley will correct the tear trough as part of your lower blepharoplasty; this correction may involve fat movement (transposition) and fat grafting.

Preparation
Preparing for surgery helps ensure a comfortable, efficient recovery. Dr. Bradley will provide you with personalized instructions, but general guidelines include:
- Stop taking certain blood-thinning medications and supplements.
- Abstain from nicotine and alcohol consumption in the weeks before and after surgery.
- Schedule time off from personal and professional commitments to focus on recovery.
- Arrange for personal transportation and assistance on surgery day and 24-48 hours of initial recovery.
Procedure
On the day of your surgery, Dr. Bradley will ensure you receive the appropriate anesthesia–typically a combination of local anesthesia and IV sedation–to keep you comfortable throughout the procedure. The procedure takes approximately one to two hours, depending on whether you are addressing the upper, lower, or both sets of eyelids. Dr. Bradley carefully places incisions in strategic locations to ensure any ensuing scars are virtually invisible once healed.
Through these incisions, he carefully sculpts tissue, removes or repositions fat, and closes with fine, specialized sutures to ensure smooth healing. You will rest in a recovery suite for a brief monitoring period before you will be cleared to return home.
Recovery
During the first few days of recovery, it is normal to experience some bruising, swelling, and mild discomfort, which you can manage with prescription medication and intermittent cold compresses. Elevate your head, especially when sleeping, to minimize swelling. You may find your eyes dry or sensitive to light, for which we can provide medicated eyedrops.
Many patients feel comfortable returning to light, non-strenuous work after 7 to 10 days, when makeup can mask residual swelling or bruising. You must avoid strenuous activity, bending over, or heavy lifting for the first two weeks to keep your blood pressure stable and minimize swelling.
As the weeks progress, you will see the bruising fade and the final shape of your eyelids emerge. While the full resolution of swelling can take several weeks, a more rested look is usually apparent quite quickly.
Results
The results of blepharoplasty are often described as a “refresh” rather than a total change. When you look in the mirror, you should see the same person, just looking as if you have enjoyed a very long, restorative vacation. As the swelling subsides and the tissue heals, your eyes will appear more open, the skin will be smoother, and the overall impression will be one of youthfulness and vitality. FAQ
Will I have visible scars after a blepharoplasty?
Dr. Bradley places incisions in the natural contours of your eyes, such as the fold of the upper eyelid or within the lashline on the lower lid. As these areas heal, the scars become virtually invisible and are hidden by your natural anatomy.
Is the blepharoplasty procedure painful?
Anesthesia will keep you comfortable during surgery. In the days following, you may experience some tightness or mild discomfort, but most patients find this easy to manage with prescribed medication and ice packs.
When can I wear my contact lenses again?
We typically recommend waiting about two weeks before wearing contact lenses to avoid irritation or pulling on the eyelids while they are in the sensitive healing phase.
